1. All Congress Registration Form must be filled completely. Partially completed registration forms will not be accepted. All forms should be forwarded via mail or fax to the following address.

2. An acknowledgement note with a confirmation number will be issued to you via email or fax only within 14 days of receipt of this official ICSC 2009 Congress Registration Form, Please state your email address and fax number clearly on the form.
3. All registrants are entitled to a delegate bag, full convention kits, an ID tag and a goodie bag.
4. Please retain a copy of this form for your record until the receipt is sent to you. An Official Invitation Letter will also be sent to you, together with the Programme Brochure.
5. Alternatively, participants can register 'on-site' at the registration counters.
